FORM 18. INTERROGATORIES

    Pursuant to Rule 33 of the Utah Rules of Civil Procedure, plaintiff requests defendant to answer the following interrogatories in writing and under oath, within 30 days after service.

    INTERROGATORIES

    INTERROGATORY NO. 1: Please identify all persons who witnessed the accident which occurred on _______, which is the subject of this action.

    INTERROGATORY NO. 2: Please identify all doctors or other medical personnel who have treated you for any injuries
sustained as a result of the accident which is the subject of this action.

    INTERROGATORY NO. 3: Please identify all facts which support your contention that defendant caused the accident, which is the subject of this action.

    INTERROGATORY NO. 4: Please identify the amount of all damages which you claim were caused as a result of defendant's actions.

    INTERROGATORY NO. 5: Please identify all jobs you have held during the last two years including the name of the employer, the title of the position you held, and the amount of salary or wages you were paid while in that position.
