Plaintiff alleges:
1. Plaintiff is a resident of _______ County, Utah.

3. Venue is proper in this court because this action involves a contract to be performed in this county.
4. On _______, defendant executed and delivered to plaintiff a promissory note under which defendant promised to pay to plaintiff on demand the sum of $_______, with interest at the rate of _______ percent per annum. A copy of the promissory note is attached hereto as Exhibit A.
5. Defendant is in default under the note and owes plaintiff $_______ in principal, plus interest from _______ until the note is paid.
6. Under the terms of the promissory note, plaintiff is entitled to recover attorneys' fees and other costs of collection.
